Leave of Absence
If your health care provider takes you off work, you must provide medical certification to your manager supporting your need to be off work.
If your health care provider takes you off of work for 5 or more shifts:
- Provide your manager with the medical certification supporting your need to be off work.
- Request and complete the leave of absence paperwork
If you are unable to return to work on or before the day your leave of absences expires, you must provide your manager additional written medical certification supporting your continued need to be off work.
At your next appointment ask your health care provider if you have any work abilities. Provide updated work abilities to your manager after each medical visit.
If released to return to work with restrictions, ask your manager if temporary transitional duty is available.
Long Term Disability (LTD) benefits may be available, contact UNUM for details.
Salaried employees should contact UNUM for salary continuation benefits if out of work for over 7 days.
