Your safety and good health are important to us. The first step to your recovery is your commitment and full participation in the process. Recovery begins with you.

Report all injuries immediately to your manager.

Communicate with your health care provider. Attend all scheduled appointments and follow up with all authorized medical treatment, physical therapy and diagnostic testing.

Keep in touch with your manager and advise them of any work status changes. After each appointment provide your manager with written updated work abilities.

Sedgwick Claims Management Service (SCMS) is Costco’s third-party administrator that will administer your workers’ Compensation claim.

It is Sedgwick’s responsibility to make certain you receive all benefits to which you are entitled under the Workers’ Compensation Law. The claims examiner at Sedgwick will handle all aspects of your claim. They will evaluate your claim for benefits and will pay all medical bills and wage replacement benefits that result from an allowable on the job injury or occupational disease. You may contact them for additional information including claim determination, approval of medical care, return to work or payment of benefits.

Your manager can provide you with your claim number and your Sedgwick examiner’s contact information.

If your health care provider takes you off work, you may be eligible for workers’ compensation benefits to replace a portion on your average wages. You will want to communicate any work status changes to your Sedgwick examiner and your manager as soon as possible.

Your Sedgwick examiner can assist you with answering questions about your claim such as benefits, physicians; including referrals to specialists, return to work and other claim related issues.

Medical Management

Bunch & Associates is Costco’s medical management company for your workers’ compensation claim.

In some cases a nurse case manager from Bunch may be assigned to your case and may contact you to discuss your injury. The nurses at Bunch are experienced medical case managers who will assist you in understanding your medical condition and treatment.

The nurse will work with you, your health care provider, Sedgwick and your manager to assist you with a speedy recovery and return to work.

The nurse may also help you with scheduling appointments with specialists, physical therapists and tests such as MRIs, CT scans, etc., as recommended by your health care provider. The nurse will also assist you in understanding your physical capabilities to ensure a safe return to work when it is medically permitted.

Provider Bills

Bunch and Associates is also responsible for processing payment for all provider bills. Should you have any issues with outstanding bills, please contact Customer Service at (888) 853-4735, Option #6.

Aetna Workers Compensation Prescription Drug Program – AWCRx

Aetna administers the Costco’s workers’ compensation prescription drug program for Costco employees with Workers’ Compensation claims. This program allows you to obtain your prescriptions quickly with no co-pay or out-of-pocket expense to you. A temporary AWCRx First Fill letter will be provided to you by your warehouse manager at the time of claim filing. This temporary First Fill letter will allow you to receive the first supply of medications while waiting for you permanent AWCRx card.

You should receive your permanent AWCRx card within 7 days of claim acceptance by Sedgwick. This card should be used to fill any future workers compensation prescriptions.

Workers’ compensation prescriptions can be obtained at your local Costco Pharmacy. If a Costco Pharmacy is unavailable a local pharmacy may be located by calling Aetna at (800) 404-2349.
