Employment Status Change or Life Event

You may be eligible to make changes to your benefits and enroll eligible family members when you have experienced an employment status change or a life event.

Employment status changes include a change from salaried to hourly, full-time to part-time, part-time to full-time, transfer from one state to another. When you experience an Employment Status Change a Benefit Enrollment Worksheet will be mailed to you from Costco to your home address. If this packet is returned to Costco as undeliverable, we will forward it to your location.

Life Events are the changes that only you know about – having a baby, marriage, divorce, death, gaining other medical coverage, losing your other coverage or dependents who are no longer eligible. In this case a Benefit Enrollment Worksheet will NOT be mailed to you. You will need to initiate a change in your benefits for Life Events.

You must access the enrollment website and enroll your newborn child, newly adopted child or child newly placed for adoption within 60 days of the date of birth, adoption or placement for adoption. You have only 30 days to access the enrollment website and make your changes if you have any other change in status or Life Event.
